Step 1:

Obtain information from the government department. To apply for information you can download an application form direct from DCS by going to:

Note: In 2015, the NSW Government changed their policies in relation to access to information about birth fathers, you may now be able to obtain his name even if he did not consent to the adoption. Contact the Adoption Information Unit below.
